Great saphenous vein (Long saphenous vein)
Vena saphena magna
- Origin
- Arises from the medial aspect of the dorsal venous arch of the foot anterior to the medial malleolus
- Insertion
- Ascends the medial leg and thigh to pierce the cribriform fascia at the saphenous opening, draining into the femoral vein (saphenofemoral junction)
- Function
- Longest superficial vein in the human body, providing superficial venous return from the medial lower extremity
- Innervation
- Sympathetic perivascular plexus
- Movement & chain
- Courses within the saphenous fascial compartment; subject to high hydrostatic pressure in upright posture
- Palpation
- Readily visible and palpable anterior to the medial malleolus and along the medial thigh
- Clinical
- Varicose veins due to saphenofemoral junction incompetence, superficial thrombophlebitis, and venous stasis ulcers (medial gaiter region)
- Segmental innervation
- Superficial lower limb venous system

- First-choice conduit for coronary artery bypass grafting (CABG) and peripheral femoropopliteal bypass; treated via endovenous thermal ablation (EVLA/RFA) or foam sclerotherapy
