Fibular artery (Peroneal artery)
Arteria fibularis (peronea)
- Origin
- Largest branch of the posterior tibial artery arising from the tibioperoneal trunk
- Insertion
- Descends along the medial crest of the fibula embedded within flexor hallucis longus to the lateral ankle and heel
- Function
- Supplies the lateral and deep posterior leg compartments, fibula bone, and lateral malleolar/calcaneal networks
- Innervation
- Sympathetic vasomotor fibers
- Movement & chain
- Protected deeply against the fibular shaft
- Palpation
- Not individually palpable due to deep muscle investment
- Clinical
- Frequently spared in advanced diabetic atherosclerotic disease ('run-off vessel' for distal bypass); vascular pedicle for vascularized free fibula transfer
- Segmental innervation
- L5–S2 spinal segments

- Workhorse donor vessel for free osseous fibula flaps in mandibular reconstruction and major limb salvage bone grafting
