- What you examine
- Integrity of the calcaneofibular ligament, and with greater tilt also of the anterior talofibular ligament.
- Procedure
- Supine, ankle in neutral. Fix the lower leg and tilt the calcaneus with the talus into inversion. Compare the excursion and end feel with the other ankle.
- Positive when
- More inversion than on the other side, with a soft end feel.
- What the test is worth
- Sensitivity around 0.50 and specificity around 0.88 in the acute phase. After four to five days, once the swelling has settled, both improve.
- Caution
- In neutral you mainly test the calcaneofibular ligament; in plantar flexion the test shifts to the anterior talofibular ligament. Record the position you tested in.
