- What you examine
- Strength and pain of the supraspinatus, tested with the thumb pointing up, with less load on the biceps tendon than the empty can.
- Procedure
- Arm at 90 degrees of abduction in the scapular plane, thumb up. Apply downward resistance while the patient holds the position.
- Positive when
- Loss of force or recognisable pain compared with the other side.
- What the test is worth
- Comparable values to the empty can (sensitivity around 0.77, specificity around 0.68) but more comfortable, which makes weakness easier to judge.
- Caution
- Distinguish pain from weakness. Weakness without pain weighs more towards a tear than pain without weakness.
