FADIR test
FADIR, anterior impingement test, flexion-adduction-internal rotation test
- What you examine
- Femoroacetabular impingement and labral injury.
- Procedure
- Supine. Bring the hip passively into 90 degrees of flexion, then adduction and finally internal rotation, noting when the complaint appears.
- Positive when
- Recognisable pain deep in the groin, often indicated with a C-shaped grip around the hip.
- What the test is worth
- Sensitivity high (0.90 to 0.99), specificity low (around 0.10 to 0.25). Suited to ruling out: a negative test makes impingement unlikely.
- Caution
- A positive test says the hip is the source, not which structure. Labrum, cartilage and capsule all give the same response.
