Tendon sheath of tibialis anterior
Vagina tendinis musculi tibialis anterioris
- Origin
- Begins ~3-5 cm superior to the superior extensor retinaculum on the anteromedial distal tibia
- Insertion
- Passes through the compartments of the superior and inferior extensor retinacula to end near its insertion on the medial cuneiform and 1st metatarsal base
- Function
- Facilitates low-friction gliding and nutritional support for the tibialis anterior tendon during ankle dorsiflexion and foot inversion
- Innervation
- Sensory articular branches of the deep fibular nerve
- Movement & chain
- Glides back and forth dynamically with every gait cycle
- Palpation
- Palpable along the anterior-medial ankle; palpable crepitus during active dorsiflexion
- Clinical
- Tibialis anterior tenosynovitis (crepitant tenosynovitis in runners and hikers); pain and palpable crunching under the extensor retinaculum
- Segmental innervation
- L4, L5 spinal segments

- Activity modification, cold therapy, non-compressive shoelace tying, and eccentric strengthening of dorsiflexors
