Origin

Ankle, plantar foot, and toe phalanges I–V

Insertion

Distal phalanges of all toes; includes synovial sheaths of EDL/FT, EHL, FDL, FHL, fibularis longus, digital annular/cruciform pulleys, and extensor hoods

Function

Provides low-friction synovial glide channels and mechanical pulley restraint for extrinsic flexor and extensor tendons of the foot and toes

Innervation

Tibial (plantar nerves) and deep fibular nerves (L4–S2)

Movement & chain

Coordinates digital flexion, extension, and push-off propulsion

Palpation

Palpable along retromalleolar courses and dorsal toe surfaces

Clinical

Flexor hallucis longus (FHL) stenosing tenosynovitis ('dancer's tendinitis' / trigger toe); tibialis posterior tenosynovitis; hammer toe deformities.
