Synovial Sheaths of the Toes
Vaginae synoviales digitorum pedis
- Origin
- At the level of the metatarsophalangeal joints, where the sheaths surround the long toe flexor tendons
- Insertion
- Continuing to the attachment of the flexor tendons on the distal phalanges
- Function
- Reduce friction and hold the long toe flexor tendons (flexor digitorum longus and flexor hallucis longus) against the bone during gliding
- Innervation
- No motor innervation of its own; sensation of the surrounding tissues via the plantar nerves
- Movement & chain
- Move passively with tendon gliding during flexion/extension of the toes; no contractile function of their own.
- Palpation
- Palpable along the plantar aspects of digits II–V during active motion; normally smooth and non-tender.
- Clinical
- Flexor tenosynovitis of the toes, trigger toe stenosing tenosynovitis, and infectious closed-space tenosynovitis following puncture trauma.
- Segmental innervation
- None (no muscle tissue); sensory supply via the medial and lateral plantar nerves, S1-S3.
- In clinical practice
- Diabetic foot inspection and pressure ulcer prevention; monitor for friction points from footwear or bedding and ensure correct anti-embolism stocking fit.
