Fibularis longus
Musculus fibularis longus
- Origin
- Lateral condyle of the tibia and the head, proximal two-thirds of the lateral fibula
- Insertion
- Base of the first metatarsal bone and the medial cuneiform bone (plantar side)
- Function
- Eversion of the foot, supports the transverse arch of the foot and assists plantarflexion
- Innervation
- Superficial fibular (peroneal) nerve, L5-S1
- Movement & chain
- The tendon runs beneath the foot and pulls the first metatarsal bone downward, giving it a key role in stabilizing the arch of the foot during push-off.
- Palpation
- Readily palpable proximally around the fibular head; the tendon can be followed behind the lateral malleolus.
- Clinical
- Overuse causes symptoms on the lateral lower leg in runners; peritendinitis occurs behind the lateral malleolus.
- Segmental innervation
- L5-S1, via the superficial fibular nerve; part of the myotome testing pattern for eversion of the foot.
- In clinical practice
- Diabetic foot inspection and pressure ulcer prevention; monitor for friction points from footwear or bedding and ensure correct anti-embolism stocking fit.
