Transverse Head of Adductor Hallucis

Musculus adductor hallucis, caput transversum

Origin

Plantar metatarsophalangeal ligaments of the third, fourth and fifth toes

Insertion

Lateral side of the base of the proximal phalanx of the hallux, together with the oblique head

Function

Adduction of the hallux; supports the transverse arch of the foot

Innervation

Lateral plantar nerve (S2-S3)

Movement & chain

Draws the metatarsal heads together, helping to maintain forefoot width and the transverse arch during push-off.

Palpation

Deep transversal band across metatarsal heads III–V; not directly palpable through the thick plantar skin.

Clinical

Binds the anterior metatarsal heads together; insufficiency leads to splayfoot and Morton's interdigital neuroma.

Segmental innervation

Lateral plantar nerve, S2-S3.

In clinical practice

Diabetic foot inspection and pressure ulcer prevention; monitor for friction points from footwear or bedding and ensure correct anti-embolism stocking fit.
