Van der Wurff test cluster
- What you examine
- Whether the sacroiliac joint is the source of the pain.
- Procedure
- Perform five pain provocation tests: distraction, compression, thigh thrust, Gaenslen and sacral thrust. Count how many reproduce the familiar pain.
- Positive when
- Three or more of the five tests positive.
- What the test is worth
- Sensitivity around 0.85 and specificity around 0.79 against a diagnostic block. Individual SI tests fall far short of those values; the cluster is why they are done together.
- Caution
- Rule out a lumbar cause first. A radicular picture or a discogenic complaint can make the same tests positive.
