- What you examine
- Whether a radiograph is needed after a knee injury.
- Procedure
- Check the five items: age 55 or over, tenderness at the head of the fibula, isolated tenderness of the patella, inability to flex the knee to 90 degrees, and inability to take four steps.
- Positive when
- One or more items present: a radiograph is indicated.
- What the test is worth
- Sensitivity close to 1.00 for a clinically relevant fracture; specificity around 0.50. Designed to rule out, not to rule in.
- Caution
- Four steps count even if the patient limps. The rule does not apply with reduced consciousness or with distracting pain elsewhere.
